What we collect
When you submit an appointment request we collect your name, email, phone number, preferred contact method, what you need help with, an optional preferred time, and the search that led you to the clinic. Searching the directory requires no account and no personal information.
Who sees it
Only the clinic you contacted. We do not sell, share, or resell your information to other clinics, marketers, or lead brokers.
How long we keep it
Appointment requests are retained so we can resolve delivery problems, and are deleted on request.
Your choices
Email us at any time to have your submitted information deleted from our records.
